We are excited to announce Art Flores is joining the team, check out his bio by selecting his name. Art was a former student of Harmony, and we are so excited he decided to join us. He has spent the last year working in a community clinic south of Chicago. He is a kind, supportive and empathetic human, and we think he will make a great addition to the RMH team.

  • After booking, we will reach out to you via email, about insurance verification and intake paperwork.

    For some insurance panels, we use a 3rd party biller called Headway.

    You may receive an email from Headway after booking to confirm insurance information.

    Select the appropriate appointment type.

    New patient tele- if you want virtual

    New patient- For in-person.

    Please reach out with questions.

  • Clients without insurance, or are choosing to pay out of pocket.

    Select New Patient - Self-pay.

    If you want to be seen Virtually- select New Patient- Tele- Self-Pay

    Initial appointment- $300

    Follow up- $150

  • Required Next Step!

    -After Booking you will receive a welcome email with instructions.

    -You must complete intake paperwork.

    -Submit copies of insurance cards

    -Submit a copy of government-issued ID cards

    -This is required within 48 hours of booking.

    -Please contact the clinic if you have trouble with paperwork.

    -If you do not complete these steps we cannot save your appointment slot.

    -If you cannot make your appointment please cancel ASAP.

    -If you do not call us ahead of time and or No-Show for your first session, we reserve the right to refuse services.

  • We require 24 hour Notice for cancellations.

    No-show or canceling with less than 24 hour notice incurs a fee.

    New Patient- $ 250

    Follow up- $ 150
